Your report MUST include:

▪ A brief summary of the hazard, including its location
▪ What could happen if the hazard was not controlled
▪ How the workers could be protected from the forklift
▪ What item of PPE could be worn to improve the worker’s safety

  1. Quickly assess the area to make sure it is safe to proceed
  2. Ensure everyone is safe
  3. Call for help
